REPORTS TO:  Charge Nurse & Health Services Director


The Resident Assistant works under the direction of the Licensed Nurse on duty. The Resident Assistant is assigned to residents by the licensed nurse and assists residents with activities of daily living based on the resident’s Health Services Plan.

JOB STATUS:  Full-time, Hourly,11:00PM-7:00AM



Resident Care

  • Receives report and accepts assignments for resident care from nursing supervisor.
  • Assists or reminds residents with personal care based on the residents’ Health Services Plan, including but not limited to:  Transferring, Grooming, Bathing, Dining, Toileting, Mobility and Dressing.
  • Answer all pages for resident assistance in a timely manner.
  • Transports residents as needed to and from other departments, dining or activities and events.
  • Serves meals to residents in the dining room or their apartment as needed.
  • Obtains or performs vital signs, height and weight as directed by nursing supervisor.
  • Provides medication reminders per the instructions and schedule provided by the licensed nurse.
  • Reports changes in the resident’s condition and needs to the licensed nurse as well as the oncoming shift during report.
  • Maintains resident confidentiality in provision of care, documentation and communication.
  • Documents and reports care provided and resident observations.
  • Be knowledgeable and protect resident rights, treating residents, families and powers of attorney with respect.
  • Adheres to Universal Precautions and Infection Control procedures.

Resident Assistance

  • Assists with personal laundry and/or linens as needed or assigned.
  • Maintain residents’ units in a sanitary manner including changing soiled bed linens, gathering and removing garbage, attending to surfaces exposed to blood borne pathogens and reporting soiled furnishings and flooring.


  • Communicates effectively with all members of the interdisciplinary team.
  • Assist with Activities in the absence of an Activities Staff Member as requested by that department or by the residents.
  • Actively participates in educational opportunities including meetings, conferences and in-services as requested to meet regulatory requirements and the needs of the residents.
  • Assist in maintaining a safe environment for the residents and staff by reporting malfunctioning equipment, observing and reporting safety and fire risks and cleaning up spills.
  • Reports any observations of inappropriate actions or potential unusual occurrences by residents, vendors, guests, families, staff or external personal service providers.
  • Be fully knowledgeable on the facilities policies and procedures.

  1.  Must have a high school diploma or GED
  2.  Must have current Nursing Assistant Certification in the state of Indiana
  3.  A minimum of one year experience is preferred
  4.  Previous experience working with seniors in assisted living or skilled nursing facility is preferred

Required Skills
Must be able to:

    • Recognize changes in resident status and communicate changes to supervisors.
    • Coordinate numerous activities at a given time, making sound judgment to prioritize tasks and make decisions quickly.
    • Make decisions necessary to maintain efficient assigned unit functioning.
    • Communicate effectively and maintain good working relations with personnel in all facility departments, other health-related individuals, and community agency personnel.

Must be willing to:

  • Work flexible hours and overtime to ensure residents’ needs are met in emergency situations

Physical Requirements

  • Stand and walk intermittently throughout the working day, as well as reaches, stoops, bends, lifts, carries and manipulates various items, supplies and equipment.
  • Able to lift and/or carry up to 50 pounds and push or pull up to 50 pounds.
  • Able to assist with resident transfers and ambulation.
